To help us create a safe, welcoming, and well-supported experience for children with special needs, we offer a few forms that parents can complete online prior to your child’s first visit. Please provide a labeled water bottle/cup, extra pull-up or change of clothes/undergarments in a labeled ziplock bag for toileting assistance, and a preferred snack if you indicated any food/dietary needs on the registration form.
